Pattern Med

My med. ins. is denying benefits on PT although I have only used 1/3 of their benefits.?

I can't understand why my physical therapy benefits are being denied by my med. insur. provider. I have 90 allowable visits per calendar year and have not even used 1/3 of those. My insurance provider did tell me I could self-refer, which is how my plan works. Now I have almost $1600 of charges they are denying, and it took them over 3 months to notify me of this. How can I resolve this on my own. This same situation happened to me last year when I was seeing a chiropractor and I ended up paying the chiro approx. $800, even though the claims were appealed (and denied). Seems to be a pattern here. I don't know how to handle this on my own. I am a senior citizen (not Medicare eligible) and my yearly income is just slightly above poverty level so hiring a lawyer is not within my means. Can anyone help?
